This notice describes the emergency request form provided by Emergency Tree Referral Network.

Information collected

The form collects your name, phone number, damage ZIP and optional address, incident type, whether anyone is in immediate danger, optional details, contact consent, and optional damage photos. Request timestamps, referral source, landing page, and advertising attribution parameters may also be recorded. A hash of your IP address is used to limit abusive submissions.

How information is used

Information is used to review your request, contact you by phone or text only about that request, plan potential service, and document emergency work. No marketing consent is requested. Sending a request does not authorize work or confirm dispatch. The form does not request payment-card information and is not monitored by emergency services.

Storage and access

Requests and photo attachments are stored privately with access restricted to authorized staff. Photos are accessed through time-limited links. Please do not upload sensitive documents or photos of people when they are not necessary to assess the damage.

Service providers

The website uses hosting, authentication, and secure storage providers. Request integrations for GoHighLevel and StormView are prepared but are not connected unless configured. If activated, they may receive request details for service coordination.

Retention

A specific retention and deletion schedule has not yet been finalized by the owner. Contact us to ask about retention or request deletion; no fixed deletion timeframe is promised here.
